All applications must be submitted with the nonrefundable fee of $35. Please make checks or money orders payable to Bethel School of Supernatural Ministry Chicagoland.

PHOTO
You will need to either submit a digital photo of yourself, or mail a passport style photo.

REFERENCES
Three recommendation forms must be submitted to the school. They can be filled out online or submitted through mail.
One Pastoral Recommendation:
(If your pastor is your parent or spouse, ask another member of the church's pastoral staff to complete the form.)
Two Personal Recommendations:
These are people who know you well and can speak on your behalf regarding your character, gifts and calling. (Please do not use relatives for any of the required recommendations.)

If your recommendation forms will be mailed to us, in order to expedite the application process, we suggest providing each person with a stamped envelope addressed to:

Please follow up with these individuals to ensure they have completed and/or mailed the form to us in a timely manner.

APPLICATION DEADLINE DATE
Students are advised to apply as early as possible as the maximum number of students that can be accepted per year is limited. Preference for acceptance will be given to students who have submitted all required documents to the school office before September 21, 2011. The deadline for late registration is September 22, 2011.

ACCEPTANCE LETTER
Your application is submitted for review when we have received all necessary documents: your completed application, current "passport style" photo, application fee, and three references. Once the Admissions Committee has reviewed your application, you will be sent an acceptance letter.

DEPOSIT
Within 30 days of receiving your acceptance letter, you are required to submit a non-refundable/non-transferable $100.00 deposit. This assures us that you are coming to school and therefore, we are holding a place for you.
